Survivors include her husband, Danny Perkins of Lake Wales; sons, Cory Dale Perkins of Lake Wales; John Robert Hazelwood of Davenport, Iowa; mother, Ruby Helen Barrett of Thomasville, GA; brothers, Scott Barrett of Thomasville, GA; Ron Barrett of Thomasville, GA; Jeff Barrett of Pennsylvania; and 4 grandchildren.
No services, cremation is scheduled.
